Initial System Access Form

The request for initial system access must be completed and submitted to Marian Hansen. Investigators will recieve login information granting access to eSirius.

eSirius is a web based system for managing the submission, review and approval of protocols concerning the use of non-human vertebrates as well as the requisition of such animals. In the future, it will also encompass invoicing and census management. All researchers have access to a personal eSirius home page providing them with a single point of access to select IACUC and Animal Facility related information.

Alternative Search Guidance

A search of alternatives to proposed animal use (the Three R's: Replace, Reduce, Refine) is required for certain research.

This document includes information that must be included in the Alternative Section of the IACUC protocol. Examples of alternative searches for a teaching protocol, alternative searches for a research protocol, alternative websites and record keeping assistance are provided.

Exemption from Standards

Justification of Animal Numbers

• Procedure

Justification of the number of animals proposed to be used on a protocol must be provided.

The attached procedure is intended to provide guidance to investigators on how to fulfill the requirement of justifying the number of animals utilized on a protocol. It further seeks to clarify when a statistical justification is necessary, and provide examples as to when it is not. For investigators utilizing statistical justification, additional resources are available at Cornell (http://www.cscu.cornell.edu) or on the web (University of Iowa Stats Page) or (http://statpages.org).

Pain Level Classification of Protocols

The IACUC will use pain classification levels defined by the US Department of Agriculture based on anticipated pain and distress of proposed procedures.

Protocol Amendment Form

To submit an amendment to an existing protocol access eSirius and choose Submit Ammendment Form for an Existing Protocol and choose the protocol to amend. Refer to the eSirius instruction manual for guidance.

Reportable Incident Form

Fill in this form to report non-compliant incidents in your facility.

Facility Inspection Form

As one of its functions, the IACUC will make semi-annual inspections of all animal facilities. The Facility Inspection form will be used as a checklist for all facilities inspections.

Continuing Review Form

Access eSirius. Protocols due for continuing review will be displayed in the "Alerts - Annual Review Section". Refer to the eSirius instruction manual for guidance.
